	Mandatory Permission and Release If, in an emergency situation, it is necessary that you leave your child in the care of another parent at Heritage, this mandatory form must be filled out and returned to a Heritage Board Member. At no time is any parent to leave children on the premises of HHSW/EEFC without Board Member approval of this signed form. It should never be assumed that a childís care has been provided for until this form is completed.
